  • Where is OPTVida located?
    OPTVida is located 1034 W. Rsi Dr., Unit 120 Logan UT, 84321. It is in the building known as 10th West Plaza. Located 2 buildings north of Gossner Foods. We offer both in-person and telehealth services to accommodate your needs.
  • What is person-centered mental health care mean?
    Person-centered mental health care means that your treatment plan is tailored specifically to your unique needs, preferences, and goals. At OPTVida, I recognize that every individual is different, so I take the time to understand your life story, experiences, and challenges.   • What lab tests might I need?
    There are various laboratory tests to support your treatment and monitor your progress. This may include routine blood tests, hormone level assessments, and other diagnostic tests as needed. Lab tests are typically ordered during your appointment based on your specific needs and treatment plan. Once a test is ordered, you will receive instructions on where and how to complete the testing. I will ensure you have all the information you need to proceed with the lab work.
  • What paperwork do I need to complete?
    The paperwork to be completed will be sent via email used at time of booking appointment needs to be completed prior to the initial appointment. The paperwork consist of: · Practice Policies · Informed Consent · Privacy Act/HIPPA compliance · Clinical Intake questions
  • Why do I need to fill out paperwork prior to appointment?

  • What is your cancellation policy?
    I require at least 24 hours' notice for cancellations or rescheduling. Late cancellations or no-shows may incur a fee.

  • How do I get my medications prescribed through OPTVida?
    After your evaluation, I will prescribe the necessary medications and guide you on how to take them. We’ll also discuss any necessary lab tests to monitor your progress.
  • What if I don't want to take any medications?
    It’s completely normal to have concerns about taking medications, and at OPTVida, we respect your preferences and choices. My approach to mental health care is personalized, which means we work with you to create a treatment plan that aligns with your comfort level and goals. If you’re hesitant about medication, we’ll explore other options together, such as psychotherapy, lifestyle changes, and holistic treatments. Medication is just one of many tools available for mental health care, and it’s not the right choice for everyone. I'm here to provide guidance, answer any questions you have, and help you make an informed decision that feels right for you. My priority is your well-being, and will support you in finding the best path to achieving a healthy, balanced life—whether that includes medication or not.
  • What is OPTVida's refill policy?
    To ensure you never run out of your prescribed medications, I ask that you request refills at least 5-7 days before your medication runs out. You can request a refill by contacting my office or through the patient portal Please note that refill requests are processed during regular business hours. For certain medications, especially controlled substances, a follow-up appointment may be required before a refill can be authorized.
  • What is nutrition support, and why is it important?
    Nutrition support involves guidance and resources to help you improve your overall well-being. Proper nutrition is crucial can enhance your mental health, support medication effectiveness, and contribute to overall physical health. At OPTVida, I offer nutrition support that is evidence-based to help you achieve a balanced diet that compliments your mental health treatment.
  • How can nutrition support benefit my mental health?
    Nutrition plays a significant role in mental health by influencing brain function, mood, and energy levels. A well-balanced diet can help stabilize mood, reduce symptoms of anxiety and depression, and improve cognitive function. My nutrition support aims to provide you with the tools and knowledge to make dietary choices that support your mental well-being.

  • Do I need to see a nutritionist to receive nutrition support?
    While seeing a nutritionist can be beneficial, you do not necessarily need to see one to receive nutrition support. At OptVida, I provide initial and ongoing nutritional guidance as part of my mental health care services. If you require more specialized or ongoing nutrition counseling, we can refer you to a qualified nutritionist or dietitian.
  • How often will I receive nutrition support?
    The frequency of nutrition support depends on your individual needs and treatment plan. I’ll work with you to determine the appropriate level of support based on your goals and preferences.
  • What if I have dietary restrictions or special needs?
    If you have dietary restrictions or special needs, please let me know so I can tailor your nutrition support to accommodate them. Whether you have allergies, intolerances, or specific dietary preferences, i’ll work with you to develop a plan that meets your requirements while supporting your mental health goals.
  • How do I access my medical records?
    You can access your medical records through your secure patient portal. If you need assistance, please contact my office.
  • What if I need urgent support outside of office hours?
    For emergencies, please call 911 or go to the nearest emergency room. For non-urgent issues, leave a message, and I will get back to you as soon as possible. If you or a loved one are expressing suicidal ideations or having suicidal thoughts, contact the National Suicide Prevention Hotline immediately at 988.   • Does OPTVida accept insurance?
    Yes, I accept most major insurance plans for my psychiatric services. Here is a list of the major insurance's that are accepted: · Select Health · Regence/BCBS · United Healthcare · Aetna · Cigna · UMR · Optum · EMI · PEHP · DMBA · University of Utah Health Plans Within each of these insurances there are different plans please contact if you have questions about specifics.
  • What payment options are available?
    I accept various payment methods, including credit/debit cards, HSA/FSA cards, and online payments through my secure portal.

    A self-pay patient is someone who chooses to pay out of pocket for healthcare services rather than using insurance. This can be an option if you don’t have insurance, if your insurance doesn’t cover certain services, or if you prefer not to use your insurance for privacy or other reasons.
